RF CG Rooftop/S? Hold Attitude/S? placement query

Regarding Rooftops on 1 1/2 level Factories in RF CG. In one entry rule, it states that Factories without Rooftop Access points (white squares in Factory locations) are SSR contingent, and I cannot find an SSR or RF/RB CG rule that states such Factories have Rooftops. Yet, there is an illustration in the RF section that implies (using 1.5 level Factory in an example) as having them. Seems to me they should. What is the answer? And, if yes, where are their RT access points? Every hex? At the corners? Random?

S? placement includes shellholes, which are, of course, unconcealed terrain. When in Hold attitude, are these immediately revealed (S? Advance Attitude are revealed/activated by rule, if <16 hexes in open ground or performing concealment loss activity), or remain as S? counters till otherwise activated (by proximity or other Defensive Fire activation parameters)?

Thirdly, when placing S? counters in 'built-up' conditions, like the RF HASL CG, can I assume 'Factories' are considered 'built-up' and therefore roll on chart A9e for each factory hex? If so, what about roofless factory hexes that contain only one location (or can I assume rule S 19.41 Rules modification, 4.1 which states Idle chit Reserve S? placement ignores any result other than ground level, and apply that to Advance Attitude S? placement as well, in particular as it relates to roofless factory hexes? OR, do you simply place an S? in every ground level location, Idle OR Advance, when there is only a single location in that hex, as opposed to a building with multiple levels, and therefore NOT roll on chart A9e?) I am confused, and again, while I can 'make-up' my own variation, I would appreciate a rule or interpretation from experienced players.

It's the map that is built-up, not the buildings. All the multi-level buildings on the RF maps use Table A9e for placement, and because they are typically stone, will always get one S? in at least one Location in each hex.

1.5 level factories have access point where the roads enter the factories.

S? in Open Ground stay concealed until they do something that warrants concealment loss (Concealment Loss/Gain table). The exception is 5/8 counters in Open Ground locations, those are revealed when in LOS.

Roofless factory hexes are still stone VPO Locations, so automatically get a S?

Idle or Advance should not matter for S? placement (except for the depth of placement behind the frontline).
