1. If No Quarter is in effect is there ANY circumstance when a unit WILL surrender?
2. Does anyone know of an article/comment-apart from the Red Barricades rules example-that explains forming the perimeter?

1. No, but capture through CC still works.
2. I don't know, but there have been Series Replays on CG.

Specific CGs have specific ways of determining set up perimeters. KGP for example uses 2 hexes from a controlled strategic location as a potential set up area determination (of course other factors may influence this as well: overlapping perimeters, isolation, etc.).

I read A20.3 No Quarter as only allowing Low Crawl or Interdiction to avoid surrender, so my interpretation is that an Encircled unit that is adjacent to a KEU will still surrender. (But then, I might very well be wrong)

It won't.

A20.21:
"...units faced with No Quarter (20.3) never surrender via the RtPh method;..."
