That’s confusing. If we ignore the bullets as just formatting and read it as a sentence then the punctuation (note the semicolons) means that all three conditions must be met.

So to avoid ammo shortage (or to alleviate it) the Americans need to clear the Fauville map and the area surrounding the joint of the N13 and the SA map and link up a setup area between SA and Fauville.

Footnote 15 explains the motivation for the ammo shortage rules and also implies it would be all three conditions.

Do isolated units get ammo shortage in SME? I could find a reference in the CG rules.

Not automatically....IIRC, American units/weapons can be subject to it, but for other reasons than just being Isolated.
