The circumstances where your infantry in an upper level might not be hit on a WP ATT shot are pretty limited ... concealed and out of LOS. Are there others?

That would require 2 or more fortifications. But some defenses might need it. Not sure why you would smoke your own gun though.

Drifting Smoke that only affects upper levels - granted pretty/very rare occurence though.

Only a cursory mention of WP - what modifiers would there be firing WP at a moving AFV with passengers. Do you not have to hit the moving vehicle to effect the passengers or just hitting the Hex would be enough? Given that WP tends to pillar and leave gaps in the resulting smoke screen, the need to use the Size modifiers makes sense to me.

An ATT shot does not have to predesignate a particular target in a hex (D.8 & C3.332) to hit that hex.
