Do both Case 1 and Case 12 apply to TH DR for a LATW fired in the AFPh? (+2 and +2 for a total TH DRM of +4)

Only Case 12 for a LATW. So an ATR would only add the +2, a BAZ/PSK would add the +2 for AFPh, and another +2 if they want/need to avoid any backblast (firing from a building).
Even though the header for the first part says: "Guns, SW Mortars, LATW, MG, and Vehicles"
Case 1 says:
1. Fire in AFPh (+3 if in building/woods) +2
(for Gun/SW Mortar/Vehicle that did not move)
I.e., it only applies to a sub-set of what the first heading says.
