If the player predesignates an AFV as the target of a FT or DC attack, is a TH DR made or do you simply proceed to the TK DR / Placement DR?

No TH DR. For the FT, just make a TK DR with the appropriate modifiers. For the DC, make the placement DR then the effects DR. You make the placement DR in the AFPh (per a fairly recent ASL q&a; I am assuming the answer for ASLSK will be the same), so you have to remember which facing the DC was placed through.
