Foxhole to Foxhole

During the Mph, an First line MMC assault moves from under an open ground foxhole to under a same level adjacent open ground foxhole. Can someone please tell me how many mf this costs in all and break down where those mf are spent -- that is how many first fire opportunities are there against the moving unit, which opportunities are potentially FFMO, and where the MMC would be placed if broken at each of the various D1F opportunities the move allows. TIA.

1 MF to enter the new hex
1 MF to enter the 2nd FH
3 MF total

FFMO applies to the first 2 MF

1 MF to enter the other FH.

It depends where it broke.
There is no special placement for a broken unit.
