We have a squad and a leader in a hex that receives a CH. RS selects the squad. So we have an 8-3 attack vs the squad and a 4 +3 vs the leader. Roll is 4 resulting in a 2 KIA against the squad. IS the leader KIAed also to fulfill the 2KIA? Or is he is considered a separate attack? The 4 +3 resulted in a PTC vs the leader.

It isn't a separate attack, but it is different results. All those units that are subject to the same result share that result. For instance if you fire using the area target type at a hex with a squad in a foxhole, an emplaced crew and gun, and a leader, a halfsquad and a hero possessing a FT in the open and roll an original three on the IFT at sixteen FP, the hero takes a 2KIA (by himself), the halfsquad and the leader take a 1KIA (i.e. random selection between them), and the squad and emplaced crew share the 3MC result. The 2KIA only killed one unit but it only affected one unit; a result cannot affect a unit that was not subject to it.
