A German 6+1 leader and a 4-6-7 squad are in the same Location.

Case A: The Germans attack a Russian HS 2 hexes away with small arms.
(a) When the leader directs the attack (i.e. to avoid Cowering), the +1 leadership DRM must be applied (A7.53). Correct?
(b) When the leader does not direct the attack, the +1 leadership DRM will not be applied. Correct?

Case B: The Germans attack a Russian HS in the same Location in Close Combat.
(c) When the leader defends and attacks with the squad and adds the 1 FP to the attack (for 5 FP in sum), he does not have to direct the CC attack and the +1 leadership DRM does not have to be applied (A11.141). Correct?

B.c. I would say correct rereading the rules but I have never played this situation. Usually directing an attack is a voluntary decision by the player and it does not seem that adding an SMC to a MMC forces the player to use the leader direction.

The cases where a leader must always apply its drm are listed in A10.72: MC, TC, Rally, Ambush, Concealment, Search Casualties, Integrity Check.

The only case where you HAVE to apply the leadership modifier in CC is when you have created a leader with that attack.
