A 11.22 says ... Provided it has not already been eliminated/captured/pinned..

Given 2 unconcealed MMC. Defender in OG in a Trench. Attacker
Advances into the Trench. During the CCPh,

there can be no ambush so the DR are simultaneous
The Attacker rolls an outright kill (but not a 2)
The Defender rolls a 2 (also a kill).

We played it that the above rule applied, both Defender and Attacker MMC
were eliminated.

Is that the right application of the rule?

That is correct; however, the original DR2 may have also created a leader which might have changed the odds.

If such a created leader were 6+1
MUST he be used and thus is adverse (+1) DRM?

Yes, he must be used. See A10.72.

I'm surprised that none have commented this yet.
The DR are never simultaneous in CC, even if the CC is simultaneous:
If the CC is simultaneous, the ATTACKER rolls first. If he rolls a 2, he gets to withdraw before the DEFENDER can roll, or he can stay and let the DEFENDER roll.
Its therefore important that the DEFENDER doesn't roll simultaneously, or before the ATTACKER. If he does and the ATTACKER rolled 2, the ATTACKER gets the incorrect benefit of knowing whether its safe to not withdraw, or whether he must withdraw to survive.
