A 19.131 Fire Group with WSs

Deos A19.131 mean that every SW (in this particualre case, two MGs) used in a Fire Group under Ammo Shortage that rolls a 12 are malfunctioned? But only one MMC, chosen by RS, ELRs?

I'm wondering because of the introductory phrase in the only sentence that refers to SWs malfunctioning under ammo shortage (per J5 errata) being 'Unit Replacement does not occur as a result of a 12 Top Hit DR, but A.11 would apply because all SW Original B#s/X#s [EXC: DC] and Ammunition Depletion Numbers are decreased by one during an Ammunition Shortage.' Should this sentence be read to mean that the reduction in B#s always applies, or only when used via a To Hit? And if more than one MMC is using an MG, are they all malfunctioned even if RS determines only one of the MMCs is replaced?

In the particular case that led to the question, a German 5-4-8 had an HMG and a 4-6-7 an LMG under Ammo Shortage and they rolled a 12, leader directed. Both MGs malf'd? Pretty clear that the MMC to be replaced is determined by RS.

What if one of the MGs was being used by a leader, who per 19.131 seems to be immune to replacement due to ammo shortage - would his MG malf as well?

A19.131 was corrected since the 1st edition ASLRB:

A19.131.... Unit Replacement does not occur as a result of a 12 To Hit DR, but A.11 would apply because all SW Original B#/X# [EXC: DC] and Ammunition Depletion Numbers are decreased by one during an Ammunition Shortage. All vehicles and Guns are considered to have a circled B# (D3.71) one lower than their inherent B# during an Ammunition Shortage.​

So regarding your SWs, ammo shortage results in their X#/B#s being lowered by 1 (as per A.11) no matter who fired them. But when more than one SW may be affected, RS is used to select which one(s) actually malf (A9.71).

SW behave as they normally do, i.e. per A9.7. This is independent of whether or not their possessing unit suffers replacement due to ammo shortage. If a firegroup of SMCs fired three MGs under ammo shortage, no unit would suffer replacement but the MGs would potentially be affected, depending on the DR and the B#/X# after applying the reduction due to ammo shortage.

The Random Selection for replacement is also independent of the Random Selection for SW malfunction. A MMC might ELR even though its SW is unaffected; its SW might be affected while the MMC is not, or both or neither might be affected.

That makes sense but I do not see the RS for SWs in the rules, at least not in A19.131

Right, because SW are covered in A9.7-9.71. A19.131 says that the B#/X# for SW of a side affected by ammo shortage is lowered from its printed number by one but does not otherwise change the rules for SW malfunction. A19.131 adds MMC replacement over and above SW malfunction.
